Submitting Applications

Health Partners Medicare is offering a $75 incentive payment to brokers who complete a Health Assessment for a 2023 enrollment of our Special (SNP HMO) plan (only available in PA) and our two New Jersey plans, Silver (HMO-POS) and Platinum (HMO-POS). The HA can be completed and submitted through Connecture.

Please Note: This incentive does not apply to new enrollments in our Prime and Complete plans. 

Once an application is submitted, you will have the option to complete the beneficiary’s Health Assessment form by clicking on the Complete Health Assessment button of the Application Confirmation page. 

Pennsylvania Plans

• Health Partners Medicare Prime offers more benefits than Original Medicare, including transportation, over-the-counter items benefit,s and prescription drug coverage, for an affordable premium.
• Health Partners Medicare Complete offers medical and prescription drug coverage, plus added benefits such as SilverSneakers, transportation, and over-the-counter items benefit, for a $0 premium.
• Health Partners Medicare Special offers health and prescription drug coverage with no out-of-pocket costs for anyone who has Medicare and full Medicaid benefits.

New Jersey Plans

• Health Partners Medicare Silver provides comprehensive coverage and generous benefits, including an over-the-counter items allowance, dental exams, eyewear, and prescription drug coverage, for a $0 premium.
• Health Partners Medicare Platinum provides comprehensive medical and prescription drug coverage, plus additional benefits not covered by Original Medicare, all at an affordable premium.
