Understanding the HealthSpring C-SNP Verification Form

The provider verification form is a mandatory part of the enrollment process. Before you meet with your client, make sure this form is ready. You may want to print it ahead of time to ensure it’s available during your appointment. Importantly, the form must be signed by a healthcare provider. This could be a physician, a physician assistant, or a nurse practitioner. Notably, the provider doesn’t need to be in-network to sign the form.

Submitting the Form

Starting July 1st, 2026, the flexibility in submitting the provider verification form increases. Not only can you or the provider submit the form, but the client can too. There are multiple submission options for convenience: fax, mail, or even through an encrypted email system. However, timing is crucial. HealthSpring must receive the signed form within 60 days from the member’s requested effective date.

The Consequences of Delay

If HealthSpring does not receive the verification form within the specified timeframe, the client will face dis-enrollment from the C-SNP plan. This situation necessitates a new enrollment process for the client, causing an unnecessary inconvenience and potential loss of coverage.
